So far I'm seeing a pattern concerning the way we sleep. I'm going to let the questionnaire remain as it is until 100 people have participated or until the new year, whichever comes first.
I might take out the breathing difficulty question as there's a close to unanimous "NO" for that answer and replace it with "Do you use a high pillow to sleep with?"
Another potential question I might add is "In your day to day job / life are you more physically active (labourer) or are you more often sat down? (desk job)"
The reason for both these questions would be, as early indications suggest, that it might be our posture over prolonged time crushing and folding on sensitive nerves running along our precordial area, much like shoving paper in to your school bag to take it out and see that it's full of creases. The paper being our bodies and the eventual crease lines representing a result of unwitting yet prolonged pressure from bad posture (kind of lost myself in that but I hope you get what I mean).
Any other suggestions at this stage?
